Job Description:
PURPOSE OF JOB :
To actively coordinate, develop and facilitate the delivery of quality diabetes/nutrition care and education to the person with diabetes, the community, nursing staff, and physicians in accordance with the National Standards.
Coordinates all aspects of the certification process by the American Diabetes Association and the Diabetes Education program.
Provides leadership and direction to the Diabetes/Nutrition Education department.
ESSENTIAL FUNCTIONS :
1.
Directs/oversees all aspects of outpatient and inpatient care and education, according to National Standards and best practices, with consideration to age-specific accommodations.
2.
Provides leadership and oversees the development, revision, and implementation of best practices, evidence based practice, related to the care of the patient with diabetes.
Performs Associate performance appraisal process.
3.
Provides education programs to patients, the community and the health care team according to hospital and National standards.
4.
Communicates with patients, families, physicians and other health care team members in achieving excellence in diabetes care and education.
5.
Demonstrates clinical proficiency and maintains a knowledge base of hospital policies and all aspects of Diabetes management.
6.
Oversees the coordination of administrative, fiscal and technical activities to ensure efficient operations Job
Qualifications:

QUALIFICATIONS :
Education:
Graduate of an approved Nurse Practitioner Program and licensed as an NP in the Commonwealth Of Pennsylvania
Experience:
Two years clinical experience, teaching experiences, both formal and informal.
Two years program development.
Certification:
Certified Diabetes Educator Board Certified Advanced Diabetes Management preferred in addition to CDEOther Skills:
Working knowledge of computer technology systems, word processing skills.
Must be flexible and able to work in a constantly changing environment.
Must be able to work with a team consisting of physicians, nurses, technicians, secretarial staff, other hospital staff and members of the community.
Recommended Skills Administration Certified Diabetes Educator Certified Nurse Practitioner Clinical Works Education Inpatient Care Estimated Salary: $20 to $28 per hour based on qualifications.
