00 an hour Job Description Case Manager Reports to:
Director of Case Management Job Summary The Case Manager is responsible for the management of assigned cases and the coordination of care with all members of the patient's community.
The Case Manager will work closely with the Clinical Services Department to ensure that all patient care goals are met and that the needs of patients, families, and community partners are met.
The Case Manager will also be responsible for coordinating care with other community partners, such as case managers, therapists, social workers, and healthcare professionals.
The Case Manager will work closely with the Clinical Services Department to ensure that all patient care goals are met.
Essential Duties and Responsibilities Assist in case management including but not limited to:
case management activities, coordinating care with other community partners, and working with the clinical services department to ensure that patient care goals are met.

Coordinate care with other community partners so that patients have access to the highest quality of services.
Collaborate with clinical services department on patient care goals and needs.
Collaborate with clinical services department on patient care plans.
Collaborate with case managers, therapists, social workers, healthcare professionals, and others in coordinating care.

Maintain accurate records of client information and documents in accordance with state regulations.
Ensure client information is up-to-date in accordance with state regulations.
Ensure confidentiality of client information in accordance with state regulations.
Ensure accuracy of client records in accordance with state regulations.
Ensure compliance with agency policies and procedures regarding confidentiality.
Perform other duties as assigned.
Required Skills:
Excellent communication skills (written and oral) required for this position; excellent interpersonal skills required for this position; excellent computer skills preferred; ability to use a computer system effectively preferred; knowledge of EMR systems preferred but not required; ability to learn new software programs preferred but not required; ability to work independently preferred but must be cooperative team member Job Types:
