Responsibilities:
Conduct initial assessments of patients' health concerns over the phone or in person, gathering relevant medical history and symptoms.
Prioritize patient needs based on severity and urgency, directing them to appropriate care resources within the organization or in the community.
Provide clinical advice and education to patients regarding self-care, medication management, and preventive health measures.
Collaborate with physicians, nurse practitioners, and other healthcare professionals to ensure coordinated and comprehensive care for patients.
Document patient interactions, assessments, and recommendations accurately and efficiently in electronic health records (EHR) systems.
Follow established protocols and guidelines for triage and clinical decision-making, seeking guidance from supervisors or clinicians as needed.
Qualifications:
RN or LPN with a current license in the state of Pennsylvania.
Experience in Primary care.
Strong assessment and critical thinking skills, with the ability to make rapid and accurate clinical judgments.
Excellent communication and interpersonal skills, with a compassionate and patient-centered approach to care.
Proficiency in using electronic health record (EHR) systems and other healthcare technology.
Ability to work effectively in a fast-paced environment, managing multiple tasks and priorities simultaneously.
